Constipation

Constipation is a common problem when your bowel movements are less frequent, when stools become hard or difficult to pass, or when you feel you have not completely emptied your bowels. Constipation is one of the most frequent problems seen in our gastroenterology private practice in Oxford, and it is often associated with other difficult symptoms like bloating, abdominal pain, pain on passing stool, rectal bleeding, and haemorrhoids.

There are many causes for constipation, and most people have tried simple remedies before coming to a specialist for help. Common causes include lifestyle factors, aspects of the diet, and a range of gastroenterology, endocrine, and structural issues that can cause the symptoms people face.

To diagnose the cause of constipation, Dr FitzPatrick will take a complete and detailed history, perform an examination, and arrange appropriate tests, all at the Manor Hospital, Oxford. Such tests may include endoscopy (gastroscopy or colonoscopy), X-rays, ultrasound tests, breath tests, or CT and MRI scans.

There are potentially serious causes of constipation, particularly if symptoms have rapidly changed or are progressive. Any significant, new change in bowel habit should be taken seriously, particularly if associated with rectal bleeding, pain or weight loss, as a change in bowel habit can be a sign of serious conditions like bowel cancer. We will arrange urgent endoscopy and scans in such situations to rule out such causes.

Dietary factors and medications (both prescribed and over the counter) can cause problems with constipation. Careful review of these factors, and the involvement of specialist dietitians can be of great help. Other conditions, including thyroid problems, endocrine conditions, and coeliac disease can also lead to constipation. Sometimes people have issues with slow gastrointestinal transit, where the whole gut moves slowly, which can be identified on specialist tests. We can use specialist medications, in combination with dietary and lifestyle changes and laxatives, to address this.

One often neglected factor causing constipation is when there is an element of pelvic floor dysfunction. Often constipation can be associated with other distressing symptoms of urgency, leakage and incontinence in this situation. Fortunately, underlying causes can be addressed with pelvic floor physiotherapy and biofeedback, combined with other approaches including surgery, to improve symptoms and quality of life for patients with pelvic floor dysfunction.
